Many years have passed since Count Georik Zaberisk, once famed as the brilliant Royal Physician of the King of Hardland, relinquished his title and moved to the country to care for his frail sister, Lillith. His life since that time has been that of a normal aristocrat, with little out of the ordinary befalling him. So when one day the King offers Georik a new position as doctor in the capital, he jumps at the chance. However, while he is away from his manor, things take a horrifying turn for his sister when the villagers suspect her of witchcraft and as a result behead her and burn her body. Devastated by the death of his sister, Georik is grief-stricken until he realizes with shock that Lillith's severed head is still alive. Knowing that she cannot live like this forever, Georik tries to restore Lillith's body by transplanting the head from one body to the next, but despite his medical genius, he cannot make her whole again. In the depths of desperation, he stumbles across the secrets of alchemy, secrets long ago forbidden. Seeing no other way to save his sister, Georik has no choice but to start his journey down this dark and dangerous path... [From wikipedia]

This game is pretty gorey
This game is set in a fantasy version of medieval Europe and everyone appears to be wearing Gothic Lolita style clothing. Count Georik Zaberisk who comes from a illustrious but cursed family used to work as a court doctor for the King of Hardland. However, he left his job and his boyhood friends to look after his sickly sister Lilith in a country village. His parents are deceased and one day while he is away, his sister is set upon by the villagers for witchcraft and she is beheaded.
Georik comes back to find that his sister’s head is still alive and tries all ways and means to keep her alive, even if it means dabbling in the illegal dark arts of alchemy and consorting with Mephistofeles.
This has gorgeous Gothic art, the men appear to be wearing makeup lol and the soundtrack is classical/operatic/Gregorian chant and gorgeous. You have to solve some puzzles to get through the game, there is a time limit for making choices and you have to search for and combine alchemy items. Pretty tricky if you’re playing the Japanese edition like me!
This explains the alchemy items but I still have to read stuff like “hydrochloric acid” in Japanese! Arrghhh…This game is pretty gorey, not for the faint of heart.
Count Georik Zaberisk: Our hero, basically a good man but falls into madness to save his sister…he also has more than one secret identity…
Lilith Zaberisk: Georik’s sister. She is betrothed to Saint Germain, an old friend of Georik’s.
Yes, she’s really a witch as accused. That’s why her head stayed alive.
Saint Germain: Georik’s bosom buddy and a scientist involved in the invention of mechanical soldiers. Has a secret identity…There’s a real Saint Germain by the way, a famous alchemist.
Michael Ramphet: Captain of the King’s Guard, bosom buddy of Georik and a righteous man. He is more than he seems…
Bruno Glening: Effeminate, evil court physician. Played by Koyasu Takehito.
Francis Dashwood: A dubious servant of the Earl of Sandwich who’s involved with the Hellfire Club. Hits on everything male. Dashwood, Sandwich and the Hellfire Club really existed. Look it up if you don’t believe me.
Jan Van Ruthberg: A mysterious blond biseinen who runs a shop dealing in contraband alchemy supplies.
Mephistofeles: A demon, he appears to have a special connection with Georik….
