If you understand Japanese, I think you need to play with the settings more. I've encountered same "problem" with DMMD, and there is a particular setting that makes the window like that. In DMMD, setting the auto aspect ratio to on makes the screen like that, so I think you need to disable that setting (if there is an equivalent) in Pigeon Blood.
I took a screen shot of the config menu and (What I believe to be) the full screen options. If anyone can help me translate it, I'd really appreciate it!
I tried both option 1 and 2 and it didn't seem to make much of a difference

That second screenshot, the one when you go into the full-screen settings for the game.

That one describes two methods for full-screen mode.
The first one (#1, which is the one you want), will automatically set the full-screen resolution to be as wide/tall as yours, while maintaining the aspect ratio. Select that one.
The second option (#2, which was currently selected at that time), will keep the game at it's native resolution of 1024x576 pixels, and just leave black borders around the image (Which is what you're experiencing). Option #2 suggests using #2 if you cannot use #1, your graphics card doesn't support anything higher or other stuffs.
If you select the first option, and it seems stretched, then your monitor needs to be set to adjust. Otherwise, I believe that's it. o.o
Edit: The option #1 does mention that it will set the resolution based on what your Videocard and Monitor supports, so both need to support it. If you're playing on a Netbook/Laptop with a small resolution, this may be the cause to your woes.
What is your current display resolution set to?
Edit #2: ... I think I recall "No Thankyou!" also having a similar issue... Try running the game in Compatibility mode for an earlier version of Windows, or something... Hmm... I'll see if I can find that NTY! thread.

I actually have it set to option #1, but it is still displaying the black border.
- Currently, my resolution is set to 1920x1080
Oh ya! I forgot my laptop was also fussy with NTY! I'll see if that solution works
edit* I cannot change the screen resolution to 1024x576, it's too small. And all the solutions from the NTY thread don't seem to be working

Can you tell if you're running an nVidia or ATI graphics chip? (Or was it Intel? I think that's what it was, right?)
You might be able to set the functionality of games' full screen in the graphic card's control panel.
...Did we already try this with NTY!? O_o I'm getting flashbacks. XD

I have an Asus ROG laptop, with an Intel graphics chip and a nVidea graphics chip, but the Intel seems to control everything (almost no settings on the nVidea)

I know Nvdia has a setting that controls the graphics too. Or you can use it's control panel to launch games with it.
AFAIK Nvidia has a monitor to show you when it uses the Nvdia GPU instead of Intel when running applications. Would you check if you are using the Nvidia GPU instead of Intel? Intel is quite limited but AFAIK, it's powerful enough to run VNs. If it runs Nvidia then you need to fix something in the control panel.
Did you try checking "Disable display scaling on high DPI settings" (Right click on Pigeon Blood Shortcut Icon -> Compatibility)? It does not work too?
Maybe graphics card needs updating.
